This form is used for name changes for minors aged 13 and under.
Who completes the form
One parent fills in the Deed Poll form. The second parent signs their consent. This must be done in front of a witness aged 18 or over.
Where to submit the form
You submit the form and the payment due along with it at a stamping office.
