Commonly Used Circuit Court forms

Form Name and No.Description

Affidavit (Template)

This form is used to outline a sworn statement setting out the facts and relevant documents to be considered by the court as part of a case.

Change of Solicitors (Template)

This form is used to notify the Court of a change of a solicitor in the case. This can be filed by any party to the case.

Defence Form 6A

This form is used to respond to a claim. It must then be served on the Claimant.

Endorsement of Service - (Registered Post) Form 1B

This form is used to confirm that a document has been sent (served) to the other party via registered post. It must be completed by the person who sent the document.

Endorsement of Service - (Personal Service) Form 1C

This form is used to confirm that a document has been handed to the other party (served via personal service). It must be completed by the person who served the document.

Entry of Appearance Form 5

This form is used to indicate that a Respondent, having been served with a case in the Circuit Court, intends to defend it.

Equity Civil Bill Form 2B

This form is used to commence a claim based on equitable grounds or seeking equitable remedies in the Circuit Court. It gives details of the parties to the case and details of the claim being made.

Notice of Motion (Template)

This form is used for a written application for an order from the court on notice to another party in the case.

Notice of Motion for Review of Judgment Form 13

This form is used by a Respondent to set aside/vary a Judgement entered by default of Appearance or Defence against them.

Ordinary Civil Bill Form 2A

This form is used to commence a case based on a breach of contract or for civil wrongs. It gives details of the parties to the case and details of the claim being made.

Personal Injuries Summons Form 2P

This form is used to commence a personal injury claim in the Circuit Court for amounts up to €60,000.  It gives details of the parties to the case and details of the claim being made.
