Form Name and No.Description
Affidavit  (Template) 

This form is used to outline a sworn statement setting out the facts and relevant documents to be considered by the court as part of a case.

Affidavit of Service - Registered Post Order 40 R.9 

This form is used to declare that a document has been sent (served) to the other party via registered post. It is a sworn statement setting out that a document has been served, where, how, when and by whom. 

Affidavit of Service - Personal  Order 40 R.9 

This form is used to declare that a document has been handed over to the other party (served via personal service). It is a sworn statement witnessed by a person authorised to do so.

Ex-Parte Application (Template)

This form is used for a court application where only one party needs to be aware of the application and be present in court.

Memorandum of Appearance in General Order 12 R.9 No.1 

This form indicates that a Respondent, having been served with a case, intends to defend it.

Notice of Motion  (Template)

This form is used for a written application for an order from the court on notice to another party in the case.

Personal injuries Summons  Appendix CC No. 1:

This form is used to commence a personal injury claim in the High Court. It gives details of the parties to the case and details of the claim being made.

Plenary Summons  Appendix A Part I No. 1:

This form is used to commence a case in which oral evidence will be heard. It also gives details of the parties to the case and details of the claim being made.

Special Summons  Appendix A Part I No. 3:

This form is used to commence a case in which oral evidence will not be  heard. It also gives details of the parties to the case and details of the claim being made.

Summary Summons  Appendix A Part I No. 2: 

This form is used to commence a case where a specific amount of money is being claimed, with or without interest. It also gives details of the parties to the case and details of the claim being made.
